Jackie Maddox, 57, of Union City died June 29, 2015, at his residence.
Services will be held Friday at 1 p.m. at First United Methodist Church in Union City.
Burial is to follow at East View Cemetery.
Visitation will be held 11 a.m. until the hour of services on Friday.
Rawls Funeral Home in Union City is in charge of arrangements.
