Disciplinary action given

A Hornbeak nurse received disciplinary action from the state Board of Nursing in May, according to a list of recent health-related board actions released Friday.
Dana E. Freeze, R.N., was disciplined for a violation listed as alleged “unfit or incompetent by reason of negligence, habits or other cause; unprofessional conduct.”
Action taken included her license being revoked, the revocation stayed, license suspended and being required to meet certain terms and conditions, according to the Tennessee Department of Health’s disciplinary action report.
